General Dental Appointments
TERMS AND CONDITIONS
Upon requesting an appointment for General Dental services at Fortitude Valley Dentist, we kindly ask that you acknowledge the following Terms and Conditions: Free consultations:- Free consults only apply to those with health insurance. For those without health cover, a small $49 consult fee applies and $50 for X-rays if needed.
- Free consults are for one problem only (i.e. a toothache). If you have multiple issues that you would like to discuss, please book a comprehensive exam for $79, or an exam, scale & clean for $229.
- If a free consult is cancelled on the day or you fail to show up, every consult thereafter will be full priced ($50) and will need to be paid at the time of booking.
- X‐rays are not included in the free consult or the comprehensive exam. If required on the day, they are charged at $50 per x‐ray. (The $229 exam, scale & clean includes two X‐rays).
- Should you attend a Free Consult & require a written report for your insurer or another company, this will incur a fee of $50. We do not provide Medical certificates unless treatment has been provided on the day.
- A $50 booking deposit is required to secure all dental appointments.
- Please provide 24 hours’ notice or more if you would like to reschedule or cancel your appointment.
- Patients who cancel or reschedule their appointments with less than 24 hours’ notice will forfeit the $50 booking deposit.
- Patients who do not show up to their appointments will forfeit their booking deposit and will be required to pay in full at the time of booking their next appointment.
